In the realm of mental health treatment, Reality Integration Therapy™ (RIT) represents a specialized approach designed to address the complex needs of individuals with Borderline Personality Disorder (BPD). Among its three core steps—Fantasy Dismantling, Separation–Re-individuation, and Whole Object Relations—Fantasy Dismantling serves as the foundational phase aimed at disrupting maladaptive thought patterns and fostering psychodynamic growth. Recent research conducted through the Integrate BPD program provides promising insights into the efficacy of Fantasy Dismantling in reducing self-harming and suicidal (SHS) behaviors. Here’s a detailed look at our study’s findings.
The Structure of Reality Integration Therapy™ (RIT)
Reality Integration Therapy™ (RIT) is a structured therapeutic approach designed specifically for BPD. It consists of three sequential steps:
- Fantasy Dismantling: This initial phase involves challenging and deconstructing distorted and unrealistic beliefs, helping individuals align their perceptions with reality. Additionally, individuals identify and begin processing their initial trauma that created the fragment in their personality in childhood.
- Separation–Re-individuation: The focus shifts to separating from harmful internal dynamics that were internalized from the narcissistic abuse they experienced in childhood and further, developing a more integrated self-concept.
- Whole Object Relations: This final step supports the restoration of healthy, realistic relationships and interactions with one’s self and others.
Study Objective and Methodology
The primary aim of our study was to evaluate the effects of the first step of RIT, Fantasy Dismantling, as administered through the Integrate BPD virtual program. Specifically, we sought to assess how this phase impacts self-harming and suicidal (SHS) behaviors in individuals diagnosed with BPD, as well as those exhibiting strong BPD tendencies.
Study Design:
- Participants: 27 subjects, including both individuals diagnosed with BPD and those with strong BPD tendencies.
- Method: Participants engaged with the Integrate BPD program over a 3-month period, during which self-reported surveys were used to monitor SHS behaviors.
Key Findings
Our results reveal a significant reduction in SHS behaviors following the completion of Fantasy Dismantling:
- For Individuals with a BPD DIagnosis: SHS behaviors were reduced by 80.0%.
- For Individuals with Strong BPD Tendencies: SHS behaviors decreased by 85.5%.
These findings indicate a substantial decrease in self-harming and suicidal tendencies as a result of engaging in the Fantasy Dismantling phase of RIT. The high percentage of reduction underscores the potential of this therapeutic step in addressing some of the most challenging symptoms associated with BPD.
Implications and Future Directions
The observed reduction in SHS behaviors is encouraging and suggests that Fantasy Dismantling can play a crucial role in mitigating these critical symptoms. However, while these results are promising, additional research is necessary to fully understand the long-term effects of RIT on SHS behaviors and other BPD symptoms.
Future Research Directions:
- Longitudinal Studies: Extended monitoring of participants is needed to confirm the durability of the observed improvements and to assess whether the reduction in SHS behaviors is sustained over time.
- Broader Symptom Evaluation: Further studies should explore the impact of Fantasy Dismantling on a wider range of BPD symptoms to provide a comprehensive understanding of its efficacy.

The preliminary findings from our study highlight the potential effectiveness of Fantasy Dismantling in reducing self-harming and suicidal behaviors among individuals with BPD and those with strong BPD tendencies.
